Credit Return Form

Use this form when Requesting a Credit Return Material Authorization (RMA) Number

This form will submit a request for an RMA#. Please do not return any equipment until we generate your RMA# and send it to you. An RMA number must be assigned before any material is returned to the Product Repair Department.

Please ensure that this RMA is in accordance with the following credit return criteria. If you are unsure, complete the form for assistance.

  • No custom or modified product.
  • Six months from date of invoice.
  • Must be unused and in the original package.
  • Stock standards and software only.
  • Only one sales order per RMA allowed
  • 25% minimum restocking fee.
  • 25% minimum restocking fee for BN Project Returns(Material from projects are only eligible 6 months from delivery and must be stock standards.)

(include all applicable dash numbers i.e. 330100-50-00 NOT 330100). Multiple different parts can be sent on the same RMA number.
